Outpatient Treatment/Case Management Services - This program provides specialized assessment, individual and group treatment, case management, outreach, consultation and education and medication management services to individuals 60 years of age or older. By working with clients and their support systems, including families, nursing homes and other social service providers, this program helps individuals maintain their independence and quality of life. Mental health and substance abuse assessment and intervention services are also provided in collaboration with the Kitsap County’s Area Agency on Aging.
Day Treatment - Group and activities therapies are provided for older adult clients to help them maintain their living skills and remain socially active.
Therapeutic Adult Foster Homes - KMHS provides training and support to foster families in the community who can provide care in their homes to older adult individuals with psychiatric disabilities.
Nursing Home Liaison Team - This team consists of a part-time Psychiatrist and Registered Nurse who provide targeted on-site consultation and evaluation to Kitsap County nursing home residents with severe psychiatric conditions which place them at high risk for hospitalization. The purpose is to reduce the number of admits, bed days and involuntary hospitalizations by providing on-site diagnosis and treatment recommendations for residents' psychiatric conditions and by providing consultation and support to nursing home staff.
Medical and Nursing Services - These services provide consumer evaluation, maintenance, and monitoring of clients' need for and use of psychotropic medication. In addition, the physical health of clients is monitored to help differentiate between psychiatric problems and "psychiatric like" symptoms caused by physical complications. Side effects of medication are also monitored.
